The Centers for Medicare & Medicaid Services (CMS) released the 2022 deductibles, premiums, and coinsurance amounts for the original Medicare Part A and Part B programs, as well as on November 12th, 2021, the original Medicare Part D income-related monthly adjustment amounts for 2022.

Outpatient hospital services, physician services, durable medical equipment, certain home health services, and a number of other medical and health services not covered by Part A are all covered by Original Medicare Part B.

Each year, the Social Security Act establishes the amount of the Medicare Part B deductible, premium, and coinsurance. Original Medicare Part B members will pay a monthly premium of $170.10 in 2022, up $21.60 from $148.50 in 2021. Original Medicare Part B enrollees’ yearly deductible will be $233 in 2022, up $30 from the previous year’s cost of $203 in 2021.

The following variables contributed to the increases in the Original Medicare Part B premium and deductible for 2022:

  • Premiums are expected to rise year over year due to rising expenses and use across the healthcare system, as well as predicted increases in the level of care offered.
  • Congressional action in 2021 to significantly lower the growth in the original Medicare Part B premium, resulting in a $3.00 per beneficiary per month increase in the original Medicare Part B premium. Part B payments will be extended until 2025.
  • Additional contingency reserves have been made up due to the concerns surrounding the potential use of the Alzheimer’s drug AduhelmTM by those on original Medicare. The Centers for Medicare & Medicaid Services (CMS) initiated a National Coverage Determination study in July 2021 to explore if and how AduhelmTM and other Alzheimer’s disease medications will be covered by Medicare. It’s unknown whether identical drugs will be covered and used by Medicare enrollees in 2022 because the treatment is still in the works. While the outcome of the coverage determination is still unknown, we must prepare for the possibility of coverage for this high-cost Alzheimer’s medication, which, if granted, might result in much higher Medicare expenses.

Medicare Part A Premium and Deductible

Medicare Part A covers inpatient hospitals, skilled nursing facilities, hospice, inpatient rehabilitation, and some home health care services. Nearly all Medicare beneficiaries do not pay a Part A cost since they have worked for at least 40 quarters in a Medicare-covered job.

The Medicare Part A inpatient hospital deductible, which members must pay if admitted to the hospital, will rise to $1,556 in 2022 from $1,484 in 2021, an increase of $72. During a benefit period, the Part A inpatient hospital deductible covers the first 60 days of Medicare-covered inpatient hospital care. Beneficiaries will have to pay $389 per day for the 61st through 90th days of a hospitalization in a benefit period in 2022 (up from $371 in 2021), and $778 per day for lifetime reserve days (up from $742 in 2021). For beneficiaries in skilled nursing facilities on days 21 through 100 of extended care services in a benefit period in 2022 ($185.50 in 2021), the daily coinsurance will be $194.50 ($185.50 in 2021).

The Medicare Part A inpatient hospital deductible will be $1,556 in 2022, up $72 from $1,484 in 2021. The monthly Medicare Part D premium has been decided by a beneficiary’s income since 2011. These income-related monthly adjustment levels effect approximately 8% of Seniors with Medicare Part D. In addition to their Part D premium, these persons will have to pay a monthly adjustment based on their income. Part D premiums vary by plan, with around two-thirds paid directly to the plan and the remainder deducted from Social Security benefits. Social Security funds are used to pay all of the income-related monthly adjustment amounts for Part D. ​Medicare Part B Premium Increase for 2022 - the largest increase in the health insurance program's history

The Centers for Medicaid Services& Medicare Services (CMS) announced on November 12th that the monthly Part B premium will increase by $21.60 in 2022, the highest cash increase in the history of the health insurance program. Monthly premiums for standard Medicare Part B will be $170.10 in 2022, up from $148.50 in 2021.
  2. Part B of Medicare covers doctor visits and other outpatient services including diagnostic screenings and lab testing. The Centers for Medicaid and Medicare Services attributed the record-high premium increase to three factors.
  • The utilization of the healthcare system has increased, as has the cost of providing health care to Medicare senior members. COVID-19 care is being blamed for some of the rising healthcare costs.
  • Because of the pandemic, Congress took action in 2021 to dramatically reduce the predicted Medicare Part B premium rise for that year. The Medicare Part B premium will increase by only $3 per month in 2021. The Centers for Medicaid and Medicare Services have been required by Congress to repay the decreased premium over time, beginning in 2022.
  • CMS is putting money aside in case Medicare decides to cover Aduhelm, a novel Alzheimer’s drug approved by the US Food and Drug Administration earlier this year. The Centers for Medicaid and Medicare Services (CMS) is still doing studies to see if Aduhelm — as well as any connected Alzheimer’s drugs — will be covered by Medicare. Officials from the Centers for Medicaid and Medicare Services said that putting money aside to cover the cost of that drug “in no way implies what the coverage will be,” but that they must plan for the possibility that Medicare insurance will cover this high-cost narcotic, which is expected to cost $56,000 per year.

While the 14.5 percent increase in the Medicare Part B premium is significant, officials from the Centers for Medicaid and Medicare Services (CMS) pointed out that the Social Security (COLA) cost-of-living adjustment is estimated to average $92 per user, the highest increase in 30 years.   3. Some Medicare seniors’ premiums will be greater than the standard because these monthly payments are based on income. Medicare Part B beneficiaries who make more than $91,000 per year will be charged a higher premium, which may vary depending on their income. For example, someone filing an individual tax return with a monthly income of $91,000 to $114,000 will be taxed $238.10 for Medicare Part B. According to the Centers for Medicaid and Medicare Services (CMS), about 7% of Medicare beneficiaries will have to pay more than the standard monthly payment.

Almost all Medicare Part A recipients (99 percent) are exempt from paying a premium. Seniors who have not worked long enough to be charged their share of Medicare taxes must pay Part A premiums.
