You need to have Medicare Parts A and B and also satisfy the requirements of the SNP you want to join.

Who can enroll in a Special Needs Plan?

Anyone who is eligible for Medicare and lives in the plan’s coverage region and has both Medicare Part A and Part B because of age or disability can enroll in a Special Needs Plan (SNP). The availability of particular plans may differ from one state or coverage area to another.
